When Cas saves Dean, Dean is always the passive object. From hell, from Crowley in season 6, Cas will save Dean without taking into consideration what Dean thinks on the matter.

Dean tries to save Cas, and Cas refuses to be a passive object or let them switch roles so radically like that, (for him), makes it his own will to not let Dean save him because he doesn’t want to be saved. He still retained his own agency, “it was about will”.

idk I think this is something central and very important to their character dynamic. It’s part of his whole savior complex and his whole need for Dean to be the damsel he rescues over and over because he doesn’t know how they work otherwise.
